Knox County Health Inspections: June 2024



KNOX COUNTY, Ind. (We Are Knox County) - Here is a list of establishments inspected by the Knox County Health Department during the month of June, 2024.


Establishments With No Violations

Establishments With Violations


Fazoli’s- 2322 North 6th Street, Vincennes

1 Critical Violation

  • Section 191- Observed numerous potential hazardous ready to eat foods to be held past expiration date and have no type of date marking system.

 

Huck’s- 2816 North 6th Street, Vincennes

6 Non-Critical Violations

  • Section 295(c)- Shelving in reach in soda cooler is observed to be heavily soiled with fuzzy white and green accumulations.

  • Section 295(c)- Walk in cooler and freezer in kitchen are found to be soiled with old food debris and litter.

  • Section 243- Multiple single serve items are found to be stored directly on the floor throughout the establishment.

  • Section 295- Reach in cooler where premade sandwiches and salads are stored is found to be soiled with old food debris and dead insects.

  • Section 310- All vents and fan guard covers throughout the establishment are observed to be heavily soiled dark gray accumulations.

  • Section 431- Establishment is observed to be in need of a deep cleaning. This includes the entire kitchen area, equipment, floors, walls, coving, and shelving.

3 Critical Violations

  • Section 191-Numerous potential hazardous ready to eat foods are found to be held past expiration date and available for human consumption.

  • Section 173-Observed potential hazardous foods in walk in cooler to not be covered and protected from contamination.

  • Section 191- Multiple items throughout the establishment are found to have no type of date marking system.


Establishments With Complaints


  • Jay C Food Store - No violations

  • Subway (6th Street) - No violations

  • Old Chicago Pizza & Taproom - No violations  

  • American Table - No violations           

  • Huck’s- 1521 Hart Street, Vincennes

8 Non-Critical Violations

  • Section 295- All equipment in kitchen and lobby is observed to be soiled with old food debris and dirt. This includes handles, doors, outside equipment, and door gaskets.

  • Section 218- Reach in freezer in prep area is observed to be in disrepair and leaking water leaving standing water under and around freezer.

  • Section 295(c)- Reach in cooler where premade salads and subs are stored is observed to be soiled with old food debris, dust, and dead insects.

  • Section 243- Multiple single serve items are found to be stored directly on the floor in storage room area.

  • Section 297- Soda machine nozzles and ice chute are observed to be soiled with slimy pink and brown build up.

  • Section 310- All vents and fan guard covers throughout the establishment are observed to be heavily soiled with dust.

  • Section 295- All shelving in lobby and kitchen area is observed to be heavily soiled with spills, dirt, and old food debris.

  • Section 431- Establishment is observed to be in need of a deep cleaning. Entire kitchen area is observed to be covered with old food debris and litter, floors are observed to be heavily covered with a black build up, all equipment is found to be covered with old food debris and spills, utility mop sink is found to be soiled with litter, and soda bib area is observed to be heavily soiled with sticky spills and litter.

3 Critical Violations

  • Section 191- Multiple ready to eat food items are observed to be expired and available for human consumption.

  • Section 177- Food items are observed to be stored directly on the floor in walk in freezer.

  • Section 173- Observed potential hazardous foods in walk in cooler to not be covered and protected from other sources of contamination.


A follow up inspection was scheduled for one week later. During that exam, inspectors documented:


6 Non-Critical Violations

  • Section 295- Reach in cooler where premade salads and subs are stored is observed to be soiled with old food debris, dust, and dead insects.

  • Section 218- Reach in freezer in prep area is observed to be in disrepair and leaking water leaving standing water under and around freezer.

  • Section 243- Multiple single serve items are found to be stored directly on the floor in storage room area.

  • Section 310- Vents, ceiling tiles, and walls above food serving station are observed to be heavily soiled with thick gray and black accumulations.

  • Section 431- All flooring throughout the establishment is observed to be heavily soiled with black build up.

  • Section 295- All reach in coolers and freezers are found to be heavily soiled with old food debris.


A second follow up was scheduled. All previous violations were corrected when inspectors returned.
